Heartworm test Waiver

Heartworm testing is recommended:

1. To detect the disease in its early stages so that treatment is less traumatic for dogs.
2. To confirm that the medication is working as expected and that the owner is compliant in giving the medication.

 I understand that the Doctors at Community Animal Hospital recommend heartworm testing annually, with or without the usage of Heartgard year round. However you may sign a waiver every other year to continue refilling your pet's heartworm prevention. Heartworm testing is then required every other year.

 Heartworm preventative such as Heartgard can be purchased at a variety of online pharmacies and veterinarian’s offices. Owners should be aware that the Merial guarantee for Heartgard only stands if the product is purchased at a veterinarian’s office or on Vetcentric.com. An annual heartworm test and monthly compliance is required by Merial for the guarantee to remain in effect. The guarantee will not stand if purchased at any other online pharmacy.

I have read and understand the risk involved and choose to decline heartworm testing at this time. I do hereby release Community Animal Hospital and its staff from any liability should any disease result in my dog not being annually tested for heartworm disease.
