Heartworm testing
is recommended:

If disease is
detected early, it is much safer to treat.
Heartworms can live up to 5 years causing damage
to the heart and lungs.

To confirm that
the medication is working as expected. Though
prevention is very reliable, nothing is 100%.
Preventive Guarantee: Manufactures of
prevention will pay for heartworm treatment if your
pet should become positive. The conditions for this
guarantee are:
- Prevention is purchased from your
veterinarian or accredited online pharmacy such as
vetcentric.com
- If started after 8 weeks of age, two negative test are shown six
months apart.
- Purchase history matches medical record for year round prevention

understand that the Doctors at Community Animal
Hospital recommend heartworm testing annually, with
or without the usage of prevention year round.
However you may sign a waiver every other year to
continue refilling your pet's heartworm prevention.
Heartworm testing is then required every
other year.
I have read and
understand the risk involved and choose to decline
heartworm testing at this time. I do hereby release
Community Animal Hospital and its staff from any
liability should any disease result in my dog not
being annually tested for heartworm disease.
